Position: Florida Case Manager

Compensation: $40,040 gross per annum
Monday through Friday – 08:30 AM – 5:00PM

WHAT WILL YOU BE DOING?
We are seeking individuals who possess, or are eligible to apply for Certification as a Behavioral Health Case Manager in the state of Florida. Our Case Managers provide services such as assessment, case management, supportive counseling and housing support services to our clients. The position assists with housing location and retention. This position is exempt as determined by the Fair Labor Standards Act.
· Provides direct housing and supportive services, including the teaching of independent living skills.
· Responsible for developing, implementing and monitoring individualized service plans for assigned residents.
· Responds to crises and takes appropriate action to ensure the safety of persons served, staff and community.
· Responsible for case management of assigned residents.
· Maintains documentation and clinical records in accordance with agency policy and contractual agreements.
· Responsible for assisting clients in planning, accessing and participating in social, recreational and leisure activities.
· Responsible for promoting cooperative relationships with individuals, families, landlords and community partners.
Required Qualifications: Bachelor’s degree in Psychology, Social Work, Social Sciences, or related field, Certified Behavioral Health Case Manager in Florida, and one year experience in direct work with individuals with mental illness. Special exceptions may be made for individuals whose equivalent professional experience and/or education may be substituted in lieu of required qualifications.
Preferred Qualifications: Experience working with individuals with severe and persistent mental illness in community/residential settings; and two years of case management experience.
